Obtaining Affiliate Status At Georgia Institute of Technology

Hello!

If you are reading this, you and/or your students require access to our core facilities for longer than 6 months, and you will need after-hours access as well.  If both of these assumptions are indeed the case, you each will need affiliate status at Georgia Tech.

To start the process to set up your affiliate status at Tech, each person must call Karen Ethier at 404-385-5281 and provide her with the following information:

 

1. Please indicate if you already have a BuzzCard and a GT ID number (this action can help you save some steps in the process).

 

1.Your full legal name

2.Date of birth

3.Social Security Number

4.Name of core facility and lab manager 

5.Estimated start and end date of affiliate status

 

Given the confidential nature of this information, please do not email her these details nor leave a message on her phone, but actually speak with her – she needs this information in order to request a GT ID#.   Please note:  she is normally in the office Monday-Friday; 8am-5pm.

Once Karen has these pieces of information, she will request a GT ID#.  It may take several days before this number is generated, but once it has been, she will advise you of it so you may include it on the affiliate on-boarding forms that need to be completed/notarized and then turned in to the Office of Human Resources (OHR) here at Tech.

Please advise Karen once your forms are complete/notarized and you have made an appointment with OHR.  She will create various documents (Personal Services Form-PSF), the Affiliate IP Waiver memo, and the Background Check Waiver form) and then send these to you via email attachment so you may print these out as well and take them with you, along with the above-noted on-boarding forms, to OHR.  Once you have completed the processing of the on-boarding forms with OHR, please then go to the BuzzCard office, pay $30, and pick up a BuzzCard.  Please be sure to bring current photo ID (US passport/green card, or government issued DL and SSN, etc.) with you when you visit OHR and the BuzzCard office (other than ID the only other document needed when you pick up your BuzzCard is a hard copy of your PSF).

 

Note:  while students like to use gmail, yahoo, etc. email addresses, please ensure a university email address is used when corresponding.
